The primary purpose of the Home Support Worker-Day Program is to enable individuals to achieve and/or maintain health and personal independence in the community and to supplement the care and support available from family and friends. The Adult Day Program offers a safe, stimulating environment with meaningful programming activities which enhance socialization and self esteem. This service is provided by a multi-disciplinary team of which the Home Support Worker is an integral member. This position may be required to support Home Care at Community Hospital, O’Leary if required. Duties will include but not limited to:

  • assisting the Day Program Coordinator with implementation of activities and other events scheduled for the day, through positive interaction with the participants;
  • assessing client's skills, needs, interests and values related to leisure;
  • developing and implementing both individual and group activity programming;
  • assisting individuals with the activities of daily living including, but not limited to, bathing, dressing, transferring, meal preparation, respite and essential household tasks;
  • assessing, documenting and reporting client's progress to the Team Leader/Coordinator;
  • educating, supporting and encouraging individuals and families to take responsibility for meeting their own needs;
  • fostering a positive working relationship with clients and their families, other service providers and community agencies;
  • participating in case conferences by sharing pertinent information concerning client progress; - performing other related duties.

Qualifications

  • Successful completion of a Resident Care Worker Program, Licensed Nursing Program or equivalent as recognized by the employer;
  • This is a bilingual position. Applicants are required to be proficient in both French and -English, oral and written
  • Demonstrated ability and commitment to work effectively in an interdisciplinary team environment;
  • Demonstrated ability to be flexible and adaptable and to work independently with little supervision;
  • Performing client assessments – ie TLR assessments, and falls risk screen in collaboration with the multidisciplinary team;
  • Capable of sustained physical activity and physical ability to complete personal care;
  • Excellent communication, organizational and decision-making skills in dealing with clients and families;
  • Ability to work effectively with individuals and families in the home setting;
  • Time Management skills;
  • Stress Management skills;
  • Conflict Resolution skills;
  • Ability to utilize smart phones, virtual applications such as TEAMS, WEBEX; - Current CPR, TLR, WHMIS and hand hygiene certificate;
  • Learning Essentials and Approaches to Palliative for Personal Support Workers (LEAP PSW) is preferred;
  • Canadian Remote Access for Dementia Learning Experiences (CRADLE) is preferred;
  • Knowledge in provision of personal care and support to clients living at home;
  • Knowledge of proper Home Care Techniques including feeding and activities of daily living;
  • Knowledge of community resources relevant to home care;
  • Require computer experience with basic data entry skills.
  • Must possess a valid driver’s license and have access to a reliable vehicle;
  • Must be available to work during days, evenings, weekends, holidays and storm days as required;
  • Must be bondable/security checked;

Other

  • Recent provincial home care experience (within one year) is preferred.
  • Nonviolent Crisis Intervention will be considered an asset;
  • Gentle Persuasive Approach (GPA) will be considered an asset;
  • Professional boundaries training will be considered an asset;
  • Experience in chronic care, and/or palliative care will be considered an asset;
  • Advanced Code White training will be considered an asset;
